As of August 2025, Immigration, Refugees and Citizenship Canada (IRCC) has introduced an important change to the Express Entry system. All candidates invited to apply for permanent residence (PR) must now complete and submit an Immigration Medical Exam (IME) before submitting their electronic PR application (e-APR).
Previously, applicants were allowed to submit their PR application first and complete the medical exam later during processing. This new requirement is a decisive step by IRCC to speed up processing and improve efficiency in the system.

If you receive an Invitation to Apply (ITA):
Failure to provide proof of a medical exam at the time of submission can now result in your application being considered incomplete or refused.

The IRCC has made a very significant policy adjustment 👏. Although requiring medical checks up front may seem like an unnecessary step for candidates, doing so will ultimately save time, cut down on delays, and prevent backlogs. The most important lesson for Express Entry applicants is to move quickly: schedule your medical with a panel doctor as soon as you receive an ITA, and make meticulous plans because medicals are only valid for a year. The PR process will undoubtedly become more transparent and predictable as a result of this change.
