A Major Policy Shift by IRCC

As of August 2025, Immigration, Refugees and Citizenship Canada (IRCC) has introduced an important change to the Express Entry system. All candidates invited to apply for permanent residence (PR) must now complete and submit an Immigration Medical Exam (IME) before submitting their electronic PR application (e-APR).

Previously, applicants were allowed to submit their PR application first and complete the medical exam later during processing. This new requirement is a decisive step by IRCC to speed up processing and improve efficiency in the system.

Why This Change Matters?

The decision is aimed at:

  • Reducing processing delays by ensuring medical requirements are met upfront.
  • Preventing application backlogs, as medicals were often a bottleneck in past cases.
  • Strengthening Canada’s health admissibility screening to confirm that applicants meet public health and safety standards early in the process.

Who Needs to Comply? Apply Now

This update applies to all Express Entry applicants, including those under:

  • Federal Skilled Worker Program (FSW)
  • Federal Skilled Trades Program (FST)
  • Canadian Experience Class (CEC)
  • Provincial Nominee Program (PNP) via Express Entry

Spouses and dependent family members included in the application must also complete the medical exam.

What Applicants Should Do? Apply Now

If you receive an Invitation to Apply (ITA):

  1. Book your medical exam quickly with an IRCC-approved panel physician.
  2. Upload proof of your medical exam along with your PR application.
  3. Plan ahead for validity: medical results are usually valid for 12 months, so timing matters.

Failure to provide proof of a medical exam at the time of submission can now result in your application being considered incomplete or refused.

Preparing for Success Apply Now

  • Schedule early: Panel physicians may have long wait times, so book immediately after receiving your ITA.
  • Bring documents: Passport, ITA letter, and medical history details.
  • Check validity: If processing goes beyond 12 months, you may need to redo the exam.

Final Takeaway

This policy change in 2025 underscores IRCC’s effort to simplify the Express Entry process and speed up permanent residence approvals. While it adds an extra step for applicants upfront, it offers greater clarity and faster outcomes in the long run.

The IRCC has made a very significant policy adjustment 👏. Although requiring medical checks up front may seem like an unnecessary step for candidates, doing so will ultimately save time, cut down on delays, and prevent backlogs. The most important lesson for Express Entry applicants is to move quickly: schedule your medical with a panel doctor as soon as you receive an ITA, and make meticulous plans because medicals are only valid for a year. The PR process will undoubtedly become more transparent and predictable as a result of this change.
